Tirzepatide, a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist, has emerged as an important therapeutic option for type 2 diabetes mellitus (T2DM). While clinical trials have established its efficacy, real-world evidence describing prescribing behavior and dose utilization after market introduction remains limited. This study assessed outpatient dispensing patterns and changes in tirzepatide strength mix in Japan using nationwide administrative claims data.

A retrospective observational analysis was conducted using the National Database (NDB) Open Claims Data of Japan. Tirzepatide dispensing records were evaluated according to healthcare setting and available dose strengths over sequential reporting periods. Trends in outpatient dispensing and shifts in the distribution of prescribed strengths were analyzed to characterize prescribing practices following commercialization.

Outpatient dispensing represented the predominant route of tirzepatide utilization throughout the observation period and demonstrated consistent growth after product launch. Early prescribing was largely concentrated in lower initiation strengths, reflecting adherence to recommended dose-escalation protocols. As clinical experience increased, prescribing progressively shifted toward intermediate and higher maintenance strengths, indicating successful treatment continuation and routine dose titration in everyday practice. The changing strength distribution suggested increasing physician familiarity with tirzepatide and greater confidence in optimizing therapy for long-term glycemic management. Overall dispensing volumes increased substantially over time, reflecting expanding adoption across outpatient care settings. These findings provide one of the first nationwide descriptions of real-world tirzepatide utilization in Japan and demonstrate that prescribing patterns closely align with recommended clinical dosing strategies.

National claims data indicate rapid uptake of tirzepatide in Japanese outpatient practice, accompanied by a progressive transition from initiation to maintenance dose strengths. The observed prescribing trends are consistent with guideline-recommended dose escalation and suggest increasing integration of tirzepatide into routine diabetes management. These real-world insights may support healthcare providers, policymakers, and researchers in understanding treatment adoption patterns and optimizing future therapeutic strategies for patients with T2DM.
